The doorway is too small for the ghasts to fit through anyway.

Hahaha, i heard rumors on forums that only ghasts, CAN come out of portal! But i dont give a **** about that and its false in my opinion. Nobody has proof yet.
i had one spawn, i had to be a chunk away from it, but one still spawned, and started a forest fire around my mountain fortress... Arrows FTW ._.

Ghasts spawn all over, but only in the nether, they don't come from one focal location, and the nether is currently only accessible through single player.
