This is FlyGuy, a simple little game where you play as a fly who must dodge falling shapes while trying to fly as fast as his little fly wings can take him. We've already released the Kongregate version, and are in the process of making a complete iOS/Android overhaul with several additional modes in addition to the "simple" Kongregate version (if there is enough interest)

We've made some big changes to Fly Guy (though the project has taken a back seat to our two other projects: Physix and Collapsus... both of which will be up on IndieDB soon). The most notable changes are to the general graphical presentation, though much of the gameplay has been changed as well, you just can't see that in screenshots very well.  We should have more news soon.
