I skimmed through HL2 codes. It's CUtlLinkedList Node memory allocation/deallocation part that throws the error. So it seems memory issue.
I haven't met that error, but I'll try to recreate the error on my machine to see if there is anything we can do to prevent this with level designs.

That part, Barney should throw a combine mine into the house through the window, then the mine bounce to the barrier, breaking it. So we want to know whether:
- The mine is in the house, but it doesn't move
- The mine is not there, Barney failed to spawn it
- Throwing event itself didn't happen properly

My advise was assuming that Technicha is in the first chapter. So it wasn't suitable for your part... I could have said keep staring "the" right one, instead of "look right"

Leave the numbers. The objects on the ground will fit to the place thanks to... Well, you can see each piece will be pinned if you are doing right, and you can see the square on the wall will be filled by orange piece by piece. Good thing is, pieces won't go out from upright - you just have to think about thier rotations(pick one and turn), then push/place them.
If you are refering to the weapon-less part, it is driven by uncontrollable force. There is one rule, though. I think the hint text explains it.
But the title is there for a reason.
