1. If it falls out, you just can't see it.
2. It depends on your mod. This is possible.
3. It keeps 60 fps on about 10 animating ragdoll at the same time.The ragdoll turn to "sleep state" when it stop animating. It cost low CPU.

I'm curious about the implementation of the func_physbox. Does it use any third-party physics engine like bullet or physx? Can share the source code with us? I'm now doing some work about the ragdoll on goldsrc.
And xashXT0.81 could not load the old version xash.dll.
Could you please give me a sign about where the animation code lands on? I want to know the code that controls every frame of the model( like scientist) bones. Is that data structure in memory similar to in smd file? My grammar is not that good, but you probably know what I mean. Thanks a lot.
