Although a commonly propagated idea, this is a myth for several reasons. Conventional armor of any type will suffer the same fate as a vehicle that uses ERA if hit in the same spot twice. While a projectile with less penetrating power than say an APFSDS round, such as a standard RPG-7 hitting ERA will trigger the explosive in what may be considered overkill, an extremely lucky successive follow up shot on the same spot will simply be absorbed by the composite armor underneath.

If an APFSDS round (for the sake of the scenario) hits the exact same spot twice on ANY MBT in the world, you are absolutely 100% f*cked

Nope, it's not a myth but it is a commonly known fact that ERA modules work only once, since they can not "respawn" after they detonate. Of course it isn't a problem for the T-84 Oplot since it has multiple layers of ERA modules which give it better protetion and endurance to resist more attacks, than conventional ERA does which has only one layer.

And of course, hitting the exact same spot twice is very unlikely in realistic combat, only a tandem-charge warhead can do that, but not APFSDS penetrators. I was merely pointing out that each ERA module will disappear after detonation and then the basic armor will be exposed.

For APFSDS penetrator hitting the exact same spot twice is very unlikely in realistic combat due to the tank's movement and random deviation of the firing gun hit probability. MBTs that are covered by ERA will lose some of their ERA modules when hit in combat and then their basic armor will be exposed. But that is of course still better than not having any ERA modules in the first place.

Of course a single ERA brick only works once, I never said they magically re-materialized after having been destroyed. A successive hit on the same depleted ERA brick in a real war scenario is almost impossible given any type of weapon, not just APFSDS. Let me explain my scenario a little bit clearer. If you are driving a Western MBT in some Mid East low tech conflict with the standard composite armor of a Western MBT (not talking add on packages such as TUSK or the Chally 2's ERA) and you sustain multiple hits in the exact same spot (magically) by a standard PG-7 (non VL) you will have suffered the same damage as you might in say, a T-90A. This is because SSR/Russian designs also have composite armor underneath their ERA modules, not as thick as Western MBTs, but enough to stop an RPG from penetrating. Of course this is to say nothing of Afganit or ARENA on Russia's most modern designs (the former possibly able to stop even APFSDS by ensuring the kinetic rod has begun to tumble by the time it hits the tank).

While tandem warheads were (and still are) in theory a good way to counter ERA originally, the Russians have long since innovated on the basic concept of ERA since it was first introduced and this was actually demonstrated just recently when a Syrian T-90 took a direct hit to the turret cheek from a BGM-71E which is a tandem charge warhead, designed specifically in to defeat Russian ERA, it did precisely nothing.

Layering of ERA is not necessarily a fail safe, as multiple bricks can still all pop at once, Russia took years perfecting ERA's evolution to the point of Relikt, which is what I'd call an "aware" ERA in terms of knowing when and when not to detonate. The T-14 uses a combination of APS>ERA>NERA>Composites just for reference. If I recall correctly the Ukrainian indigenous ERA is equivalent to up-specced Kontakt-1 (they were never given access to Kontakt-5).

Sorry I keep referencing Russian designs, but they are the most qualified in terms of Duplet armor.
