The main difference between Strategic War in Europe and Hearts of Iron 3 is that the first is turn based and second is real time.

Almost every aspect of the game is moddable. Everything depends on the skills of the modder, not only the gfx can be modded but also the gui positioning, ingame values, modifiers, combat tables, even some gameplay rules. Most of this can be easily done with a notepad.
